I have a locked down desktop on Windows 2008 R2 server.
The desktop is empty with no icons on it, users are not allowed to create new ones.
It's done with this GPO:
User Configuration\Policies\Administrative Templates\Desktop:
'Hide and disable all items on the desktop' - Enabled.

I've disabled Windows standard start menu using all available options (special keys, shift+right click etc.)
but once I double-click LMB anywhere on the desktop
the standard menu shows up.

If this GPO setting above is disabled and icons are enabled on the desktop
then Classic Start Menu works fine - it's not possible to open the standard menu.

Could it be a bug in Classic Start Menu or did I miss some option to disable this behavior?
Really appreciate any help.

This is a bug in Windows and not related to Classic Shell. You will see this behavior with the Windows Start Menu even if you exit Classic Shell's menu or uninstall it. When this policy you mentioned is enabled, double clicking the Desktop opens the Windows Start Menu.

If you don't want the standard Windows Start Menu to appear for users, then you can remove permissions for the 'Users' group from the following Registry Key;
HKEY_CLASSES_ROOT\CLSID{4622AD11-FF23-11d0-8D34-00A0C90F2719}
