The Metro button had to be removed because Microsoft had a legal complaint about it being similar to the Windows logo. You can download it here: viewtopic.php?f=18&t=802 and apply it using the Custom button option. The old style Metro button is also downloadable: viewtopic.php?f=18&t=734

Also, I don't have "applications" in the start menu (this feature was in the past above all programs)...

Right click the Start button -> Settings. You can either change to the 'Classic with two columns' style from the first tab. This was the style used in earlier Classic Shell versions. That will show Apps next to Programs.

There is also now a new Windows 7 style in Classic Shell 4.0 and later. If you decide to use the Windows 7 style from the first tab in settings, then you can enable "Apps" in the right column too. Go to the Customize Start Menu tab and set "Metro Apps" to "Display as a menu".

Go to the Customize Start Menu tab in the settings and find the Apps item.

@sparker781, if you are using the classic with two columns style, go to Customize Start Menu tab and drag the "Apps" item below the column break so it appears on the right. Position it where you want and click OK. For Windows 7 style, Apps is already on the right. Just enable it to display as a menu.
